Please be aware that a privacy breach related to e-mail correspondence is possible. Reports can be faxed or directly mailed.
Forms: Please complete and sign the adult patient form, if a child then child form. Included on the adult patient form are informed consent, release of information, patient-psychologist agreement and credit-debit policy. Please bring the completed and signed forms to the office for your appointment (twelve pages).
